A New Wrinkle Reducer Set To Debut This Spring

Come this spring, you may have yet another option to choose from in terms of injectables that erase, and prevent, wrinkles. Xeomin, a new botulinum toxin from the makers of Radiesse and Asclera, just received approval by the FDA to temporarily improve the appearance of lines between the eyebrows (glabellar lines).

While it’s still unknown how Xeomin will be used differently than Botox or Dysport, we do know that Xeomin is formulated as a more pure form of botulinum toxin without any additives, possibly making it less likely to cause adverse reactions. Xeomin is also the only FDA-approved botulinum toxin that does not require refrigeration. Xeomin is currently used in 14 countries including Germany, the UK, France, Italy and Spain, under the name Bocouture.

SUNSCREEN - the most effective anti-aging ingredient


You’ve all heard me lecture about the importance of using proper sun protection. Nothing is more proven than proper protection from ultraviolet light.

Sunscreen is the most effective anti-aging ingredient on the market today.

An effective sunscreen is the best way to maintain a youthful appearance, prevent immunosuppression, and avoid skin cancer. Though it was once believed that UVB radiation was the only wavelength harmful to the skin, evidence now suggests that the UVA radiation is more significant regarding long-term effects on the skin. For full protection against the sun, it is essential to use a sunscreen that protects against a broad range of both UVA and UVB radiation. Unfortunately, 99.99% of the sunscreens available in drug stores and even department stores, are chemical based, meaning they only protect against a small portion of the sun’s damaging rays. Only titanium oxide and zinc oxide are proven to effectively block ALL of the UVA and UVB rays. The problem is, most of the chemical-free sunscreens are thick and white and gloppy – not the look you’re going for! Sun damage is responsible for up to 90% of the visible signs of premature aging on the human face. In addition to wrinkling, visible blood vessels, skin roughness, pigmentation changes, and skin discoloration, the long-term consequences of photoaging include dramatic loss of skin elasticity and thinning of the skin. This is the result of ultraviolet radiation’s direct effect on the collagen matrix, which is the ‘scaffolding’ that gives firmness and strength to the skin. When exposed to ultraviolet radiation the collagen folds on itself, and as this happens, the skin thins, and wrinkles and furrows appear.

The Facts of Light:

  • The skin is the largest organ in the human body.

THE NON-SURGICAL "NOSE JOB"

At COSMETIC LASER MD, we often come across patients who ask about “non surgical nose jobs,” “injectable rhinoplasty,” or “liquid rhinoplasty.” In the last ten years, we have seen explosive growth in minimally invasive and non surgical procedures. There are many reasons that people are opting against “the knife:”

• Little or no downtime
• Much less expensive
• Little of no discomfort
• No anesthesia
• The ability to refine the area exactly the way you would like it to be

As a result, the various fillers have seen an enormous increase in their utilization. And today, we use these various fillers to achieve younger, more symmetric appearances not just by filling in wrinkles but also by restoring lost volume (the liquid face lift). More recently some of these fillers, which have only been approved by the FDA for use in the nasolabial fold, are increasingly being used for an ever wider array of “off- label” procedures including reshaping the jawline and chin, filling the cheeks, and more recently to reshape the nose, i.e. the “non-surgical” nose job, “liquid rhinoplasty,” and “injectable rhinoplasty.”

The Non Surgical Nose Job is an alternative to surgical rhinoplasty for many individuals who wish to alter the shape of their nose or restore symmetry to their nose. The treatment takes only about fifteen to thirty minutes to perform, unlike traditional rhinoplasty which is done under IV sedation or general anesthesia with several weeks of down time, a relatively painful recovery, and severe bruised for several weeks. The non-surgical nose job only requires the application of local numbing cream for about 20 minutes.

Unlike a rhinoplasty where the patient is unconscious, with this procedure, the patient is an active participant in the procedure. The patient has a mirror in his or her hand and can help guide the procedure.

Radiesse (Dr. Tanne's usual filler of choice for the nose) is injected to fill depressions, smooth out sharp angles, or change the angle of the tip of the nose, restoring symmetry and making your nose appear smaller and more attractive. With Radiesse there is no need for allergy testing. Radiesse stays exactly where you place it. In other words, it does not migrate like some hyaluronic acid based fillers. This is extremely important for injection into the nose. Radiesse lasts about 1 year which is twice as long as most other fillers.

Botox can also be used as part of a nonsurgical nose job to relax the muscle that pulls the tip downward, creating a more upturned appearance, or to soften the "bunny lines" on the sides near the top of your nose.

Dr. Tanne likes to use Radiesse for simultaneous chin augmentation. This is a complementary procedure to the non-surgical nose job. Radiesse is FDA approved for injecting into laugh lines and restoring volume to the face. Other uses include injection into marionette lines, and augmenting your cheek bone structure to restore a more youthful look. Some refer to this as the non-surgical face lift.

Dr. Tanne has performed literally hundreds of “non-surgical facelifts” and is one of the experts in this growing field. In addition, she has had many patients specifically asking for “non-surgical nose jobs” and is quickly becoming the area leader in this procedure as well. Ideal Candidate for the Non Surgical "Nose Job"

• Patients who wish to diminish the appearance of bumps on their nose.
• Patients who have dents, hollowness or indentations, or have noses which look asymmetric.
• Patients who have a flat nasal bridge, and wish to create a bridge or augment their original bridge. This is a very common request in the Asian and African American population.
• Patients with droopy tips or downward appearing tips who wish to raise the tip of the nose.
• Patients who have had surgical nose jobs in the past and are unhappy with the appearance of their noses.

A commonly asked question is whether this procedure can actually make one’s nose look bigger. By taking away nasal irregularities and creating more symmetry through nasal contouring, the nose actually looks smaller and more proportional to one’s face. The nose serves as a central focal point when we look at a person’s face. When you restore symmetry back to this important structure in the middle of the face it actually looks smaller. If you create a taller nasal bridge, the nose appears much thinner.

A REALITY TV star has revealed the truth after her fresh face and narrower nose at an event sparked speculation that she had undergone plastic surgery.

Rather than using a filler to tackle individual smile lines, the dermatologist said that she lifted Zarin's face by injecting Restylane along the jaw line and up along the sides of her face. She is considered one of the premier specialists in this procedure, and has performed several hundred of them.
"It’s a little uncomfortable, but definitely not painful at all," says Zarin, 47. "But it's 100 percent worth it. She took ten years off my face." Zarin said she's been getting Botox in her forehead and between her eyes for 10 years - and more recently, in her neck. "I'm scared I'm going to get a turkey neck," she said. "It runs in my family."
As for her new nose, Zarin says it's not needles or knives, but rather the work of her new hair and makeup artist, Jim Crawford. "He put brown contouring makeup on my nose and thinned it out," she says. Crawford explains the makeup magic: "We also made her face look thinner by airbrushing the sides and hairline two shades darker," he says. "Before she was just using TV makeup, which didn't give any dimension."
Getting rid of bangs also changed up Zarin's look.
"With the bangs down, the nose takes up the largest percentage of her face," Crawford said.

The Liquid Eyelift


Out of the popularity of the “liquid facelift” comes another option to rejuvenate your appearance. This one is being called the “liquid eyelift.” To set the record straight, neither one is a true lift because the results are temporary. However, for those individuals who are looking for a non-surgical rejuvenation, the “liquid eyelift” can be a welcome option.

Using injectable fillers (such as Juvederm, Restylane and Radiesse), Botox or Dysport, and eMatrix, Dr. Tanne rejuvenates the entire eye area, giving you a refreshed, more youthful appearance. Filler is used for delicate crinkly skin under the eye to reduce the appearance of hollows and lines, and to gently re-inflate the sunken area in the temple. Crow's feet are erased with Botox/Dysport and a bit of filler. EYES WIDE OPEN


In the quest for younger-looking eyes, nonsurgical rejuvenation options take you from fatigued to fabulous

The eyes. There is nothing more powerful on your face: They cry, they laugh; they show disappointment, dreaminess or deceit; they reveal surprise and shock, happiness and honesty. Beyond expressing how you feel, they can even define who you are. With all the importance we place on the eyes, it seems like a cruel twist of fate that they exhibit signs of aging so soon.

“People form opinions of other people based on their eyes, more than any other area on the face,” notes Dr. Susan Stevens Tanne, Medical Director of Cosmetic Laser MD. “People assume that if your eyes look young, it implies that you have more energy, more spunk, and are more of a go-getter.”

You may feel perky, but if your eyes look droopy, tired and sad, your whole demeanor can appear fatigued, depressed and old—all characteristics that appear on the daily “don’t” list. Unlike other facial imperfections, which can be camouflaged with artful makeup, problem areas around eyes are tougher to hide. Makeup and facial creams can’t cover up wrinkles and creases caused by volume loss under the eyes and in the temples.

Lighten Up!
Dark circles definitely aren’t a “do” in the quest to maintain youthful-looking eyes. First, you need to determine the cause. As the skin thins with age, underlying blood vessels may show through, appearing as dark circles. Sometimes the pigment of the skin actually changes color as red blood cells break down. In other cases, those circles are actually dark shadows. As facial skin sags and loses volume, the area above the cheeks (the tear trough) hollows out, causing a shadowed effect. And sometimes dark circles have nothing to do with age—you just have them and probably always did, just like your parents and siblings.

Easy fix: An excellent concealer can do the trick for some.

Medical fix: If the dark circles are caused by shadows, injectable fillers in the under-eye hollows can make them disappear. "I get beautiful results with hyaluronic acid fillers like Juvéderm and Restylane, as well as with Selphyl enhanced fat from one’s own body,” says Dr. Tanne. "These fillers can fill the hollow areas beneath the eye, restoring that smooth youthful volume."

Tighten Up!
Your facial skin and muscles weaken with age—and that includes the skin above and below your eyes. The eyelids stretch, causing the upper lids to droop, while fluid and/or fat begins to fill in the loose space below the eyes, creating bags and puffiness.

Love your Lashes

Celebrity Cosmetic Surgery Red Carpet Secrets Revealed

With the Oscars a distant memory, the only question that now remains is – how do the celebrities get themselves so PERFECT before they walk the red carpet?

After all, millions of eyes and flashbulbs capturing every move on the red carpet, and with it, any flaw that dares to bulge, spot or wrinkle on the world's most glamorous stage.

“Though we did not treat any Oscar-hopefuls this year, we have certainly seen our share of Broadway, television and movie personalities here at Cosmetic Laser MD,” says Susan Stevens Tanne MD.

“There are a variety of nonsurgical 'quick fixes' that have little or downtime that I can choose from to make them look good. Celebrities have always been at the forefront of using these kinds of treatments, because of the pressure to constantly look good for public appearances," Tanne adds.

Ideally, patients come in a few weeks before their big day to assess their concerns, and put together an action plan of what is often a variety of touch-up procedures to tighten skin and give it a glow.
So what kind of fixes are celebs clamoring for? Here’s a list of the top most requested pick-me-ups.

FAT SHRINKAGE
When you've got to slip into a sexy body-baring gown, Spanx can only work so many miracles. In addition to supportive undergarments, celebs turn to the VelaShape contouring system to trim a few more inches of fat.

CHEST EXPRESS

Years of sun worshipping sometimes takes its toll on the upper chest, which is spotlighted when wearing a strapless gown.

BROWN SPOTS

Entirely eliminating brown spots takes multiple treatments or more invasive procedures like deep chemical peels that require downtime, and can't be done merely days before a big event. Powerful lasers may zap problematic spots into oblivion, but they form a scab that must naturally flake off on its own, or else risk scarring.

However, a variety of noninvasive treatments like microdermabrasion and non-ablative lasers can be combined like a cocktail to significantly lighten spots in as little time as a week before the big day.

For maintenance, a regular patient may first go through the regimen of four to six sessions to undo sun damage on the skin, and then maintain with follow-up sessions once every season.

FILLERS AND BOTOX
Botox is often used before an event to eliminate crow’s feet, forehead lines, and other fine lines and wrinkles. It is easily our best “quick fix” and our patients are thrilled with the lineless, youthful results they can achieve.

Fillers like Juvederm and Restylane are most often shot into the lips for a fuller look, or to erase aging marionette lines. Radiesse is a firmer filler, and is often used in the cheeks for more support. While typical fillers last a few months, firmer versions like Radiesse can often stay in place for up to a year.

Sold by the syringe, most patients require two syringes of filler for an area like deep laugh lines, or one syringe for the lips. Prices can vary dramatically depending on volume needed.

When it comes to Botox and fillers, you have to be careful to treat at least a week before an event, just in case there’s any surface bruising. It is difficult to anticipate whether or not a person will bruise, but eliminating blood thinners like aspirin helps.

Advanced Facial Sculpting: Beyond Filling Lines


Advances in filler technology have enabled aesthetic physicians to greatly change the way they treat patients. At Cosmetic Laser MD in Livingston, New Jersey, Susan Stevens Tanne, M.D. uses volumizing fillers such as Radiesse and Juvederm Ultra Plus to move facial structures such as cheeks and jaws. “Juvederm Ultra can augment the hollow under the eyes and the lips,’” says Dr. Tanne.
The nasolabial fold (the crease the extends from the corner of the nose to the mouth) forms not from the lack of soft tissue in this area, but rather from MID FACE DESCENT - that slow march south of the mid face that begins in one’s 20’s when bone loss and drooping of the fat pads combine with skin laxity to allow the face to drift downwards. The result of this is a line that deepens into the nasolabial crease.
While some filling of the crease will help improve this, much of the nasolabial crease can be corrected by injecting filler into the upper cheek. By using structural and volumizing fillers, a true “face lift” can be attained.
“Injections to lift the face are among the best uses of fillers that I can offer to my patients. Don’t allow yourself to be treated by someone still trapped in the “line filling” approach,” says Dr. Tanne.
Filler injections into some other areas also help reverse the signs of aging, and only need a small amount of product for great results. For instance, filler injected into the earlobes will correct the creases that form in the ear lobe and the sagging that occurs. Filler will easily reverse this. The replaced volume frequently provides dramatic result. The nose is an area that droops with aging. Small amounts of injected filer will give a “lift to this area”. The back of the hands are another area that shows signs of aging. Volume correction gets rid of the “bony and veiny” appearance of this area. The use of laser photorejuvenation to eliminate brown spots, and fractional resurfacing to improve the texture of the skin, completes the rejuvenation of this area.
As new techniques and products become available, the art of using injectable fillers is being elevated beyond what some may regard as mere “spackling”.

Photodynamic Therapy - PDT - for Acne and Sun Damage



What is Photodynamic Therapy?
Photodynamic therapy (PDT) using Levulan® is a very safe and effective treatment for moderate to severe acne, aging, and severe sun damage.
At Cosmetic Laser MD, Susan Stevens Tanne, M.D. offers Photodynamic therapy with Levulan® and the BLU–U® light or FotoFacial RF (IPL) from Syneron to help reverse sun damage and treat acne. Photodynamic therapy (PDT) is a special treatment that uses a photosensitizing agent that is applied to skin and then activated with a light source. The beauty of utilizing PDT is its ability to rejuvenate skin — in addition to successfully improving such skin conditions as acne and severe sun damage — all in one procedure.
How does Photodynamic Therapy Work?
The Levulan® is absorbed by actively proliferating cells such as bacteria, oil glands and sun damaged cells. The light source used may be a visible blue light, intense pulsed light or laser light. It activates the Levulan® in the targeted cells which are then destroyed. Bacteria are destroyed, oil glands shrink and sun damaged cells peel away. Skin texture is improved, pore size is reduced, and skin appears smoother, brighter, and healthier.
What happens during and after the treatment?
The patient experiences a mild tingling during the treatment. The next few days feel like a sunburn as the old cells are shed off.
It is very important that you avoid direct sunlight for 36 hours (including car windows) after having the treatment. Sunlight contains the spectrum of light that can unpredictably activate the chemical and result in a much deeper peel than was expected.
What conditions can be treated?
• Sun damage: face, neck, shoulders, back, chest, hands, arms and legs
• Acne, oily skin, and enlarged pores
• Rosacea
• Keratosis pilaris (tiny bumps on cheeks and the back of arms)
What happens during PDT Treatment?
Clean your skin of all makeup, sunscreen and moisturizers. If you have recurrent cold sores, we'll start an Antiviral prophylaxis. We will perform an acetone scrub or microdermabrasion. A clear painless solution is applied to your skin and left on for 30–60 minutes. It is then activated by a light source, usually BLU–U®, LightWave blue LED therapy or IPL, for a few minutes and then washed off. Most people require three to five treatments, two to four weeks apart.
The treatment will also target sebaceous hyperplasia (those small, benign bumps on your skin), and the papules of acne rosacea. Skin oiliness is decreased, and the appearance of pores is minimized. The entire skin texture improves following a Photodynamic Acne Treatment.
PDT Acne Therapy
There are several different presentations of acne ranging from comedonal (blackheads and whiteheads), papular, pustular, and cystic acne. In many cases, many of these presentations of acne can be present on an individual simultaneously.
Prior to photodynamic acne treatment, the best available treatment option for cystic acne was Accutane. However, with Accutane there are many systemic side effects including birth defects, liver abnormalities, mood depression, and virtually all patients get dryness. PDT Acne Treatment provides a viable alternative for all types of acne treatment. For many people this means being able to avoid oral antibiotics or Accutane.
FotoFacial RF and PDT
Photodynamic therapy can be combined with our regular skin rejuvenation treatments using FotoFacial RF, which targets redness and brown spots, as well as stimulates formation of more collagen in the skin. There is normally no downtime for FotoFacial RF treatments without Levulan. However, one can enhance and boost the FotoFacial RF skin rejuvenation results by adding Levulan to the treatment process. The down time is a little longer than for a typical FotoFacial RF treatment but the results are phenomenal! The pores become finer, spots flake off and the result is a subtle air brushed look that your friends and family will compliment you on.
Is there any downtime? What happens after treatment?
We'll apply sun block and make sure you wear your hat out of the office. Patients are more sensitive to the sun for 36 hours after treatment and must avoid direct light. If possible, patients should remain indoors for the first 24 hours after treatment. There may be mild swelling around the eyes for 1 day and redness and peeling for about seven to 10 days. You'll use mild cleansers and moisturizers until the peeling stops. Sunscreen is mandatory. Topical acne medications can be used in between the PDT treatments once the redness has settled down, usually over a period of a few days. Again, if you are prone to facial cold sores, Dr. Tanne will prescribe antiviral medication to prevent a recurrence.
The advantages of PDT are rapid treatment of acne lesions all over the face without scarring or surgical excision. The skin appears refreshed and younger following these treatments. There are no systemic side effects.
Who should avoid Photodynamic Therapy?
Pregnant women, those who have taken Accutane® in the past three months, and those with active cold sores. Dr. Tanne will discuss whether this treatment is appropriate for you during your free consultation.
When is the BLU–U® light or LightWave LED therapy used without Levulan®?
Since the BLU–U® light and LightWave LED destroys bacteria that aggravate acne, mild to moderate acne can be improved with just the light. Treatment involves sitting in front of the light for a few minutes twice a week for 5 weeks. It is safe and painless, and can be used together with prescribed acne medication, microdermabrasion, and light chemical peels.
